Request for Giving Envelopes
Calvary Fellowship Church will provide yearly pre-numbered giving envelopes to any and all financial supporters. We will assign you a specific giving number and this will enable CFC to track your contributions and provide you with a statement at the end of the calendar year. This statement will satisfy the IRS requirements for claiming charitable contributions on your income tax forms. You do not need to be a member of CFC to obtain these giving envelopes. At the beginning of the new calendar year, another set of envelopes will be automatically available for your use. Please watch the bulletin at that time for the location of the new envelopes.

To request giving envelopes, you may download the form in MS Word, print it out and drop it off at our accounting office OR you may open the Adobe PDF file, fill the form out online and submit it electronically to our accounting office.

Automatic Debit Form
If you would like to participate in Setting the Capstone via automatic debit, you will need to fill out this form authorizing a debit from your financial institution.  You may print out the form in  MS Word format and fill it out by hand or you may download the PDF version of the form, fill it out online, and then print it out.  This form requires a signature and a voided check.  Please drop the form off at our accounting office or mail it to the following address:
